Registration and Login

You must register on the STR website (https://OpioidSTR.mimh.edu) before you can access the Medication Tracking forms. 

Click the Register link in the upper right-hand side of the screen and complete the information. You will need to create your own User Name and Password. The password has a 7-character minimum and will accept text, numbers and special characters.

  • After you complete the registration form, click the [Register] button.  You will receive a  registration confirmation email.  This email is occasionally sent to the junk or spam folder.   Please check there if you have not received the email.  If you still have not received the registration email, please contact Keith Eldridge at Keith.Eldridge@mimh.edu.
  • The system administrator will give you permission to access the Medication Tracking form within 24 hours of registration.
  • You will receive an email from the system administrator once your account has been authenticated.
  • After receiving authentication, log on to the STR website.

Guidance for Medication Tracking Form Data Entry

  • Click the "Tracking Form" menu option to start the data entry process. 
  • Before entering client information, you must first Search for an existing record.
    • For example, enter DMH ID=3000001 and click the [Search] button.  You either get the individual with ID=3000001 or the message “No prior visits for this client.”  If there are prior Visits for ID=3000001, click the link in the Search Grid to edit the information.  If there are no prior Visits, click the [Add Visit] button to add the Visit and begin entering new information.

Medication Tracking forms MUST be filled out in the following situations

  • Every time a client receives medical services through STR where a medication is prescribed or dispensed.
  • Every time a UA/UDS is conducted (for results that are not immediately available, please go back to the original entry and update with results
  • You do not need to enter client visits during which there was no UDS AND no medication prescribed or dispensed (However, if a consumer is enrolled in STR but has not ever received medications you will need to track these individuals separately for the electronic monthly survey).
  • Please use the comment box to describe any unique situations with an client (e.g. Due to results from UDS/other health concern, medication was not prescribed/dispensed)
  • Sites may enter forms as clients are seen or at minimum, on a monthly basis. Data must be submitted by the 10th of each month and include all STR clients seen between the first and last day of the previous month that received medication or were given a UDS.

Please note: If you are unsure of whether or not to add something in the comments, err on the side of extra information.

 

Questions and Answers:

Q: What if the individual has a 30-day prescription for their medication but it’s only dispensed to them 7 doses at a time? What should I put for “days prescribed” on the Medication Tracking form?

A: Enter the amount they are given at a time, so for this case you would enter “7 days.” You can include in the comments section that prescription was for 30 days

 

Q: What if the individual is not currently enrolled in Medicaid but is Medicaid eligible? Should I enroll them in STR and enter their medical visit into the tracking form?

A: Do not enroll a Medicaid eligible individual in STR unless it would significantly delay their access to care. If that’s the case, enroll them in STR and switch them later (please notify within the Comment Box on the Medication Tracking form when and why they were taken off STR). 

 

Q: Buprenorphine is on the tracking form twice. When do I check the box under the drug screen?

A: Only check the second buprenorphine box if individual tests positive for illicit use of buprenorphine.

 

Q: I don’t see the prescriber’s name in the dropdown box. What should I do?

A: Email Keith (Keith.Eldridge@mimh.edu) and he will add them to the dropdown box.
